In order to maintain a high level of patient care, our practice serves as an out-of-network provider of physical therapy services. Fast Track Physical Therapy & Sports Performance accepts most heath insurance as an out-of-network provider. A prescription is required to participate in physical therapy services.

You have two options of payment:

1. The office manager of Fast Track will submit your out-of-network claims.
2. You can choose to take advantage of a 20% discount by doing one of two things: submit claims yourself or pay out-of-pocket at the time of service. We will provide you with an invoice with the necessary codes and information most insurance companies request. Reimbursements will then be sent directly to you by your insurance company.

Call your insurance company's customer service number, and ask them to provide you with their reimbursement policy for out-of-network outpatient physical therapy. Then, find out:

1. How much is my annual deductible?
2. How much of my deductible have I met for the year?
3. What percent of the allowable charge will my insurance cover after I meet my deductible?
4. Do I need prior authorization for services?

Find out what specific information they need on the invoice, and make a note of who you spoke with. The invoice you receive from us will meet the requirements of most insurance companies.

Being out-of-network allows us to provide longer, individualized treatment sessions and maintain a high standard of care. Treatments can be dictated by the patient's need rather than the requirements of an insurance company. It allows our rates to be significantly less than in-network providers that submit the paperwork for you. Patients avoid the traditional 3-time-per-week traditional therapy with co-pay and instead receive increased quality of care because of the personalized, one-on-one time spent with a highly qualified Physical Therapist, which works to their advantage. In addition, our lowered rates are cost effective for people with no insurance, poor insurance or high deductibles.
