What To Bring
Below is a checklist of what to bring to your first physical therapy appointment:
  • Doctor’s prescription for physical therapy
  • Current health insurance card (unless paying fee for service)
  • Current ID (driver’s license)
  • Comfortable, loose fitting clothes, and sneakers. Be conscious of clothing that can expose the injured body part
  • Bring your calendar so that we may schedule several weeks in the future to give you your preferred appointment times
  • Please arrive 10 minutes early to complete required paperwork
